The following are forms that you will find useful as a member of the MTFL.

TEAM FORMS:
A Team Captain looking to re-register an existing team? If so, please complete the 'Team Registration Form' and submit it at the Annual General Meeting.

Looking to enter a new team? If so, please complete the 'New Team Roster Details Form' and submit it via e-mail to John Schiebel prior to the start of the season. We will do our best to help accommodate you.

PLAYER FORMS:
Each player is required to complete a 'Player Registration & Waiver Form' in order to participate in an MTFL game. This form must be completed and submitted along with acceptable photo identification to the League prior to playing your first MTFL game.

Individual players that are looking to join a team, can submit a 'Player Recruitment Form'. Complete the form and submit it via e-mail to John Schiebel prior to the start of the season. We will do our best to help accommodate you.
